Integrated Care Pioneers – Two Years on
To mark the second year of the pioneer programme, and the first year of the second wave pioneers, the Integrated Care pioneer team have produced ‘People Helping People – year two of the pioneer programme’, which was launched in January 2016.
The report focuses on the work of the second wave pioneers, the progress they have made as well as the learning they have developed on top of that of the first wave pioneers.
The second wave pioneers were chosen in January 2015, with 11 places focusing on improving the delivery of integrated care provision in their areas.
